Upload Vaccination Card via smart phone
Steps to take:
- Take a picture of the front and back of your vaccination card.
- Fill out the information requested:
- Employee ID, this is the same number you use to log in to Ceridian Dayforce.
- Select which vaccination status you are updating, COVID-19 or Flu
- Complete the requested information
- If updating COVID-19 vaccination status, please upload the front and back of your vaccination card.
- You can find the pictures you took of your vaccination card in “Photos” in your phone.
- If you are exempt, please upload exemption proof.
- If your location requires testing, please select 'Yes' and provide test details.
- Save & Submit.
- If you need any assistance, please feel free to reach out to your Administrative Accounting Coordinator or your Center Director.
Upload Picture of Vaccination Card via Computer
Steps to take:
- Take a picture of the front and back of your vaccination card on your phone.
- Fill out the information requested:
- Employee ID, this is the same number you use to log in to Ceridian Dayforce.
- Select which vaccination status you are updating, COVID-19 or Flu
- Complete the requested information
- If updating COVID-19 vaccination status, please upload the front and back of your vaccination card.
- You may have to email the photos to yourself from your phone so that you can upload those photos from your computer.
- If you have been granted an exemption, please upload your exemption decision form.
- If your location requires testing, please select 'Yes' and provide test details.
- Save & Submit.

Upload Scanned Vaccination Card via Computer
Steps to take:
- Scan the front and back of your vaccination card.
- Fill out the information requested:
- Employee ID, this is the same number you use to log in to Ceridian Dayforce.
- Select which vaccination status you are updating, COVID-19 or Flu
- Complete the requested information
- If updating COVID-19 vaccination status, please upload the front and back of your vaccination card.
- If you have been granted an exemption, please upload your exemption decision form.
- If your location requires testing, please select 'Yes' and provide test details.
- Save & Submit.
